Care & Services
The role, in a few words
You are the operational heart of Upchar Setu in Bhavnagar. You manage the day-to-day delivery of our home care services — coordinating with patients, families, nurses, doctors, physiotherapists, dietitians, and other care professionals to make sure every visit is scheduled, delivered on time, and properly recorded. You are the person families and professionals call when something needs to be sorted — and you sort it.
Your work. Real impact.
Manage and schedule care visits across Bhavnagar, coordinating with families and professionals so visits are delivered on time. Keep everyone informed when arrangements need to change.
Act as the primary point of contact for enrolled patients and their families. Listen to their concerns, help sort out service-related questions, and maintain clear communication throughout their care.
Coordinate daily with nurses, physiotherapists, doctors, dietitians and other care professionals. Make sure the people involved in a patient’s care have the information they need to deliver their visits.
Respond calmly to complaints, escalations and urgent situations. Coordinate with the appropriate care professionals rather than treating coordination as a substitute for clinical judgement.
Maintain accurate records of services delivered. Keep scheduling and visit information organised so care delivery can be followed and reviewed.
Monitor patient satisfaction and the quality of services delivered. Listen to families and professionals, recognise concerns, and help resolve them responsibly.
Maintain relationships with hospitals, doctors and healthcare partners. Support the growth of Upchar Setu’s services in Bhavnagar through dependable day-to-day coordination.
Could this be you?
5–7 years of experience in healthcare operations, hospital administration, or care coordination. You should be comfortable managing the practical delivery of care across multiple patients and professionals.
Nursing, healthcare administration, pharmacy, physiotherapy, or allied health is preferred. These backgrounds can help you understand the people and services you will coordinate.
Strong Gujarati and Hindi communication skills are required; English is a plus. Families and professionals need someone who can listen carefully and explain arrangements clearly.
You can manage multiple patients and situations simultaneously, remain calm under pressure, and take responsibility for getting issues sorted.
A valid driving licence and access to a two-wheeler are preferred for travel within Bhavnagar.
