Why partner with Upchar Setu

What we bring to a partnership.

Verified Care Professionals

Every nurse, MBBS doctor, physiotherapist, and specialist in our network is credentialled and verified before they ever enter a patient's home.

End-to-End Coordination

We manage the complete care cycle — scheduling, professional assignment, family communication, and progress tracking — so your team doesn't have to.

Centralised Patient Records — Shared Across the Care Team

One of the biggest gaps in home care is that each professional arrives without knowing what the last one found. Upchar Setu maintains a centralised health record for every patient — accessible to every verified professional assigned to that patient. No repeated assessments. No conflicting advice from professionals working in silos. Every professional sees the full picture.

Prevention-First Philosophy

Upchar Setu supports corrective care — post-discharge recovery, condition management, oncology support. But we're equally committed to prevention. Our annual packages and ongoing care programs are designed to help patients live better through the year, not just recover from what already happened. For your patients and policyholders, this means fewer readmissions, fewer acute events, and a better long-term health trajectory.

Family Visibility & Reporting

Patients' families stay informed through the Upchar Setu dashboard. Care records, visit logs, and health updates — all in one place.

YOUR ORGANISATION. YOUR CARE MODEL.

Who we work with.

Upchar Setu supports four types of institutional partners — each with a tailored model that fits how your organisation works.

Hospitals & Discharge Planners

Structured post-acute care pathways — from your ward to the patient's home.

The post-discharge care transition is one of the highest-risk periods in a patient's care continuum. Upchar Setu gives your discharge planning team a trusted, clinically aligned home care partner — so every patient leaves the hospital with a structured care pathway already in place.

Hospitals & Discharge Planners

THE PARTNERSHIP MODEL

Referral model — families register and pay directly with Upchar Setu. No billing complexity, no receivables risk for your hospital. Formal referral pathway and MOU available.

Post-discharge programs available as 14-day, 21-day, or 30-day structured recovery programs — matched to the patient’s clinical need and condition at discharge.

Common referral scenarios

Post-cancer treatment or hospital discharge
Support recovery at home after cancer treatment or hospitalisation — with coordinated nursing, nutrition, physiotherapy, monitoring, and doctor follow-up. We also provide ongoing home monitoring between oncology visits.

Post-surgery recovery
Structured recovery at home after hospital discharge — physiotherapy, wound care, vital signs monitoring, and MBBS doctor follow-up visits, all coordinated through a single care team.

Frail or elderly patients
Regular health monitoring and coordinated care at home — so your most vulnerable patients are never left without supervision between clinical encounters.

What the partnership looks like

  • Your discharge team identifies patients requiring post-acute home care support (complex nursing needs, physiotherapy-led rehabilitation, or ADL assistance)
  • You initiate a care transition referral to Upchar Setu at the point of discharge planning
  • We conduct a rapid care needs assessment and assign a verified, multidisciplinary care team
  • Clinical handover documentation is collected from the hospital at the time of discharge and integrated into the patient’s Upchar Setu health record
  • Care begins on day one at home — with structured protocols, visit logging, and family communication
  • When your patient goes home, we provide a structured post-discharge monitoring and multidisciplinary support pathway — and keep you informed of relevant clinical observations and escalate as required
  • Your team receives confirmation of care initiation and can request status updates at any point

Clinical context

Unplanned readmissions within 30 days represent one of the most significant quality and cost challenges in acute care. The evidence is clear: structured, supervised home care in the post-acute period reduces readmission rates, improves patient outcomes, and supports earlier, safer discharge for clinically appropriate patients.

Upchar Setu provides the post-acute care infrastructure that bridges the gap between clinical discharge and stable home recovery — across nursing, physiotherapy, physician follow-up, medication reconciliation, and ADL support.

Post-discharge care available

Skilled Nursing (wound care, IV management, catheter care, vital signs monitoring) · Physiotherapy (rehabilitation, gait training, strength restoration) · MBBS Doctor Follow-Up Visits · Patient Attendant (ADL support, 12-hour shifts) · Dietitian · Psychologist · Care Coordinator · Social Worker · Palliative & End-of-Life Care

Why hospital discharge teams choose Upchar Setu
  • Reduce 30-day unplanned readmission rates through structured, supervised post-acute home care
  • Facilitate earlier, safer discharge for clinically appropriate patients — freeing up beds and reducing pressure on your ward
  • Eliminate the information gap at care transitions: Upchar Setu’s centralised patient health record is shared across all home care professionals, reducing the clinical risk of fragmented post-discharge management
  • Provide discharge planners with a single verified referral pathway for complex cases — not an ad-hoc arrangement with individual home care providers
  • Improve patient satisfaction scores at discharge through confident, supported care transition
  • Align with clinical governance and quality improvement goals around care continuity

Partner questions.

Answers to common questions from hospital discharge planners, clinicians, and corporate leaders about partnering with Upchar Setu.

How does the referral process work for hospital discharge teams?

Your discharge team identifies patients requiring post-acute home care and initiates a referral to Upchar Setu — by phone, WhatsApp, or our referral form. We conduct a rapid care needs assessment, assign a verified care team, collect clinical handover documentation from the hospital at the time of discharge, and begin care on day one at home. You receive confirmation of care initiation.

Does the hospital or clinic receive payment for referring patients?

No. Upchar Setu operates a clean referral model — families register and pay directly with Upchar Setu. There is no referral fee paid to hospitals, doctors, or any partner. This is a straightforward care pathway — not a commercial arrangement between institutions.

How does Upchar Setu keep the treating doctor informed?

Care professionals document every home visit — including vitals, observations, and any clinical concerns — in the patient's centralised health record. Care summaries are available on request. If the care team identifies a clinical flag requiring escalation, we contact the treating doctor directly.

Which patients are most suitable for referral to Upchar Setu?

Patients who benefit most include: post-discharge patients with complex nursing needs, oncology patients between treatment cycles, post-operative rehabilitation patients, frail or elderly patients with functional decline, and patients with chronic conditions requiring supervised monitoring between clinic visits. If unsupervised home recovery creates clinical risk, that patient is appropriate for referral.

Is an MOU or formal agreement needed to begin referring?

A formal referral arrangement or MOU is available for hospital and institutional partners. For individual clinicians, referrals can begin immediately — by phone, WhatsApp, or our referral form — with no prior agreement required.

What happens if a referred patient deteriorates at home?

Upchar Setu care professionals are trained to identify clinical deterioration and to escalate promptly. Depending on the situation, escalation goes to the treating doctor, a specialist, or emergency services (112). The family is notified immediately. We keep a clear record of all escalation events and communicate the outcome to the referring team.
