Care Coordination

What they do

Your Care Coordinator is the person who holds your loved one’s entire care plan together — scheduling visits, communicating between professionals, updating the family, and making sure nothing falls through the cracks.

When you need them

  • When your loved one has multiple care needs across different professionals
  • When you’re managing care from another city — or another country
  • When you want a single point of contact for everything care-related
  • When coordination between a nurse, doctor, and physiotherapist needs to be seamless

Without coordination, home care becomes chaotic — missed visits, conflicting advice, no one keeping track. Your Care Coordinator makes the whole system work as one.

Your Care Coordinator is also the custodian of your loved one’s centralised health record — ensuring every professional on the team has access to the same complete, up-to-date picture. No professional starts from scratch. No information falls through the cracks.

Every Upchar Setu package includes a dedicated Care Coordinator — because coordination is not optional, it’s essential.

Dental Care at Home

What they do

  • Preventive care for patients on long-term medications (which can affect oral health)
  • Identifying dental concerns early before they become serious

Palliative & End-of-Life Care

What this is

Palliative care is not about giving up. It is about choosing comfort, dignity, and quality of life — for your loved one, and for your family. Upchar Setu provides compassionate, coordinated palliative care at home for patients living with serious, life-limiting illness — at any stage of the illness journey.

Palliative care can run alongside curative treatment. It does not mean the end of hope. It means adding a team whose purpose is to manage pain, support wellbeing, and make every day as meaningful as possible.

What palliative care at home includes

  • Pain and symptom management (coordinated with your loved one’s treating doctors)
  • Skilled nursing for comfort-focused medical care
  • Psychological support for the patient — and for the family
  • Social worker support to help the family navigate this chapter
  • Dietitian guidance for comfort nutrition
  • MBBS doctor home visits for medical assessment and comfort management
  • Care Coordinator to hold the entire care plan together

When to consider palliative care

  • When a loved one has been diagnosed with a serious, advanced, or terminal illness
  • When the focus of care is shifting from cure to comfort
  • When pain, breathlessness, fatigue, or other symptoms need dedicated management at home
  • When your family needs professional guidance and emotional support through a difficult time
  • When a loved one has expressed a wish to be cared for at home through their final chapter

This is one of the most difficult things a family faces. Upchar Setu does not step back when care becomes hard — we step closer. Our care team is trained to support not just the patient, but every family member walking this journey alongside them.

You do not have to navigate this alone. We are here — with professional care, with compassion, and with steadiness — for as long as you need us.

Two Things That Make Upchar Setu Different

We're here when you need us. And before you need us.

Every service we offer can be part of a recovery plan — but the best use of home care is preventing the crisis in the first place. Upchar Setu’s annual preventive programs keep your loved one monitored, supported, and healthier through the year.

Regular nurse visits catch problems early. Dietitian guidance prevents condition deterioration. Physiotherapy builds strength before a fall happens. MBBS doctor home check-ups identify risks before they become emergencies.

CORRECTIVE WHEN NEEDED. PREVENTIVE BY BELIEF.

Every professional walks in informed.

In most home care, each new professional starts from scratch — no knowledge of past visits, previous findings, or what the last professional tried. That gap leads to repeated assessments, conflicting advice, and missed warning signs.

With Upchar Setu, every professional on your loved one’s care team has access to a centralised, up-to-date health record. Every visit logged. Every note shared. Your nurse knows what the doctor found. Your physiotherapist knows what the nurse observed. Care is connected — not fragmented.

CONNECTED CARE. INFORMED DECISIONS.

One platform. Every professional. All coordinated.

Upchar Setu is not a directory of healthcare workers. It is a coordinated care platform — where every professional works as part of your loved one's personalised care team, managed end-to-end by Upchar Setu.

All professionals verified and credentialled

About our home care services.

Can an MBBS doctor visit at home through Upchar Setu?

Yes. MBBS doctor home visits are one of our core services. The doctor reviews the patient at home, updates the care plan, manages medications, and communicates findings to the family. For specialist consultations, we also offer tele-consultations with appropriate specialists.

What kinds of nursing care do you provide at home?

Our nurses provide wound care, IV management, catheter care, vital signs monitoring, post-operative nursing, medication administration, and patient assessment. All nurses are qualified and registered with the Indian Nursing Council or the relevant state nursing council.

Is physiotherapy at home as effective as at a clinic?

For most patients, home physiotherapy is more consistent and more comfortable — which improves outcomes. The physiotherapist comes to the patient, assesses them in their actual living environment, and tailors the program to what the patient can realistically do at home. For post-operative patients especially, this matters.

Do you provide care for patients at end of life?

Yes. Upchar Setu offers palliative and end-of-life care support — coordinated nursing, pain management guidance (working with the treating palliative physician), psychosocial support for the family, and dignified care at home for patients who wish to spend their final period at home.

Can a patient attendant be arranged for overnight care?

Yes. Patient attendants are available for day shifts (typically 8–12 hours) and extended care. Attendants support daily living activities — mobility, hygiene, feeding — under nursing supervision. Exact shift arrangements are discussed during the care assessment.

Are dietitian consultations available for patients on special diets?

Yes. Our registered dietitians provide home or virtual consultations for patients on therapeutic diets — including renal diets, diabetic meal plans, oncology nutrition, post-surgical recovery diets, and dysphagia management.
