Upchar Setu is a verified home care connection platform based in Bhavnagar, Gujarat. We connect families with qualified nurses, MBBS doctors, physiotherapists, dietitians, psychologists, and other care professionals — for coordinated, managed home-based medical care. Every professional is verified, every care plan is coordinated, and every family is kept informed.
Upchar Setu is for any family in Bhavnagar that needs professional care for a loved one at home — whether that’s an elderly parent who needs ongoing monitoring, someone recovering from surgery, a patient going through cancer treatment, or a family managing a chronic condition like diabetes or Parkinson’s. We also serve NRI families who want trusted local care management for parents in Bhavnagar.
Upchar Setu is live in Bhavnagar, Gujarat — and our vision is to bring this care model to major cities across Gujarat. If you are in another city in Gujarat, register your interest today. We will reach out as we grow to your area. You are welcome here, wherever you are.
No. Upchar Setu is a home care coordination platform — we bring verified care professionals to your loved one’s home. We are not a hospital, a clinic, or a nurse staffing agency. We manage the full care experience: professional assignment, scheduling, coordination, health records, and family communication.
Several important ways. When you hire a private nurse independently: there is no verification process, no coordination between professionals, no centralised health record, and no one accountable for the overall care plan. With Upchar Setu, every professional is credentialled and verified, a dedicated Care Coordinator manages your entire care team, all visit notes and records are maintained centrally and shared across professionals, and your family has a dashboard to stay informed. You get a care system, not just a person.
Registration is free and takes under 5 minutes at upcharsetu.com/waitlist. Share your name, contact details, your loved one’s age, their health condition or care need, and your location. Our team will review your application and reach out within 1–2 business days.
No. Upchar Setu uses an approval-gated model. Families register first, our team reviews the application, and packages are available to choose and pay for only after approval. This is intentional — every care plan is matched to the family’s specific needs, not sold as a generic product off the shelf.
Because home care is not a commodity. Every patient is different, every care need is different, and placing the wrong professional in someone’s home creates risk — not care. Our review process ensures we can genuinely serve your loved one’s needs, that the right professionals are available, and that your care plan is built around the actual situation — not a generic package.
Registration: under 5 minutes. Application review: 1–2 business days (we may call you to clarify details). Package selection and payment: at your pace after approval. Paid care begins within a few days of payment confirmation. For Annual Preventive Care packages, the first visit is FREE; our team will coordinate that visit with you. We move at the pace your family needs.
Your name, phone number, and email. Your loved one’s name, age, and location (currently Bhavnagar only). A brief description of their health condition or care need. The type of care you’re looking for — preventive monitoring, post-discharge recovery, condition-specific support, or other. You can add more detail after registration when our team contacts you.
You receive an approval notification via email and WhatsApp. You’ll see the care packages recommended for your loved one’s specific needs. You choose a package, complete payment securely online, and your care team is assigned. We introduce your care professionals to you before the first visit.
Upchar Setu offers 8 packages across 4 categories:
– **Annual Preventive Care:** Watch, Guard, Shield, Comprehensive
– **Post-Discharge 30-Day Recovery:** Physio Intensive, Nursing Intensive
– **Monthly Condition-Specific Support:** Chemotherapy Support Program
– **Attendant Intensive:** Vatsalya Complete — hands-on daily support at home

Every Upchar Setu package includes: verified and credentialled care professionals, a dedicated Care Coordinator managing your entire care plan, a centralised health record shared across your care team, family dashboard access (care schedule, medical records, raise a concern), mobile app access and digital health monitoring, and transparent pricing with no hidden charges.
No. The package price is the full price for the services included in that package. If your loved one’s needs change and require services beyond the package scope, our team will discuss options with you transparently before any additional cost is incurred.
Yes. Speak to your Care Coordinator or contact our team. If your loved one’s needs change — for example, moving from a preventive to a more intensive package after a health event — we will guide you through the process.
Register and tell us about your loved one’s situation. Our team will recommend the right package based on their specific health needs — there is no obligation until you choose and pay. You can also speak to us directly via WhatsApp, phone, or email before registering.
Our verified care network includes: nurses, MBBS doctors (home visits), physiotherapists, dietitians, psychologists, specialist and consultant doctors, care coordinators, social workers, patient attendants (12-hour shifts), dentists, and palliative care specialists. Each professional is credentialled, background-checked, and verified before their first placement.
Both. Upchar Setu provides MBBS doctor home visits — a qualified MBBS doctor physically comes to your loved one’s home to examine, assess, and manage their health. Virtual tele-consultations are also available for follow-ups and routine check-ins. The inclusion of in-person MBBS doctor home visits is one of Upchar Setu’s key differentiators.
Yes. Requesting a female care professional is a standard option in the registration form — not an exception. Simply indicate your preference when registering, and we will assign accordingly.
Upchar Setu aims to assign the same professional to your loved one’s visits wherever possible. Continuity matters — both for your loved one’s comfort and for the quality of care. We will always inform you in advance if a different professional needs to cover a visit.
Yes. Every care professional is introduced to your family — name, credentials, and a brief introduction — before their first visit. You will never have a stranger arrive at your door without knowing who they are and why they are there.
Contact your Care Coordinator immediately. We take professional fit seriously. If a professional is not the right match for your loved one — for any reason — we will reassign. Your feedback directly improves the care your loved one receives.
Yes. This is one of the most important things Upchar Setu does differently. Every patient has a centralised health record maintained by Upchar Setu and shared across their entire verified care team. Every nurse visit is logged. Every doctor’s note is recorded. Every physiotherapy session is documented. When a new professional sees your loved one, they see the full history — not a blank page.
Only your loved one’s assigned Upchar Setu care team and your family (through the dashboard) can access the health record. Records are maintained securely and are not shared with any third party without your explicit consent.
Yes. The family dashboard can be accessed by multiple family members — useful for joint families, or for NRI families where the patient’s child abroad and a sibling in India both need visibility. Contact your Care Coordinator to add additional family members to the account.
Your care schedule (upcoming visits, who is coming and when), your loved one’s centralised health record and visit notes, medical documents and reports (downloadable), and a “Raise a Concern” feature to flag anything that needs our team’s attention.
Our care professionals are trained to call 108 (India’s emergency ambulance service) immediately and alert the family without delay. They do not leave the patient until help arrives. Every professional placed by Upchar Setu knows the emergency protocol and has the family’s contact details before every visit.
Contact your Care Coordinator directly — via WhatsApp, phone, or through the dashboard’s “Raise a Concern” feature. For a medical emergency at any time, call 108 immediately. Your Care Coordinator can also help escalate care needs if a condition is deteriorating between scheduled visits.
Yes. Our coordinators and care professionals communicate in Gujarati. Care scheduling, updates, and coordination happen primarily over WhatsApp — the way Bhavnagar families actually prefer to stay in touch. You do not need to use any separate app or portal if you prefer WhatsApp-based communication.
The dashboard is currently in English. A Gujarati interface is being developed and will be available after launch.
Yes. Upchar Setu provides compassionate, coordinated palliative care at home for patients with serious, life-limiting illness. Our palliative care team includes nurses, MBBS doctors, psychologists, social workers, and care coordinators. Palliative care can begin alongside curative treatment — it does not mean giving up. It means choosing quality of life, comfort, and dignity.
Yes. If your loved one or your family has chosen to focus on comfort and dignity at home through the final stage of illness, Upchar Setu’s palliative care team will support you. This includes pain and symptom management, emotional support for the patient and family, social worker guidance, and nursing care. Please contact us directly to discuss — this conversation is handled with full care and sensitivity.
Yes. The Comprehensive annual package is designed with NRI families in mind. Upchar Setu’s Care Coordinator manages everything locally — professional visits, scheduling, updates — while you stay fully informed through the family dashboard and regular WhatsApp updates in Gujarati or English. You don’t need to be in Bhavnagar for care to work well.
Through the family dashboard (accessible from any device, anywhere) and through regular updates from your Care Coordinator over WhatsApp or email. If anything requires your attention or decision, our team will reach you directly. You will always know what is happening.
Your Care Coordinator is your local point of contact for non-emergency escalation. For medical emergencies, our care professionals call 108 and alert the family immediately. We can also contact a designated local contact — a sibling, neighbour, or trusted person — in addition to notifying you abroad.
Your discharge planner identifies patients who need post-acute home care support. A referral is made to Upchar Setu — via phone, WhatsApp, our referral form, or under a formal partnership MOU. We conduct a rapid needs assessment and assign the appropriate verified care team. Clinical handover documentation is collected from your team at the time of discharge and integrated into the patient’s Upchar Setu health record. Care begins at home on or after the discharge date.
No. Families register and pay directly with Upchar Setu — there is no billing complexity, no receivables management, and no financial risk for your hospital. The referral pathway is clean and administratively simple.
Fragmented post-discharge care — where each home care professional works without knowledge of what others have found — is one of the key drivers of preventable readmission. Upchar Setu’s centralised patient health record is shared across the entire verified care team: nurse, physiotherapist, MBBS doctor, and care coordinator all see the same up-to-date clinical picture. This continuity of information reduces the gaps that lead to deterioration being missed at home.
Yes. For patients who are clinically stable but require supervised post-acute nursing, physiotherapy, or ADL support at home, Upchar Setu can provide the structured home care infrastructure that makes earlier, safer discharge possible — without compromising patient safety.
Yes, as part of a formal partnership arrangement. Agreed reporting formats and frequency are established during the partnership setup — including care initiation confirmation, visit summaries, and flagged concerns that may warrant clinical review.
Via phone, WhatsApp, or our online referral form. Under a formal partnership arrangement, we can set up a dedicated referral pathway for your practice — as simple as a WhatsApp message with the patient’s name and care need. We handle everything from there and confirm care assignment to you.
Yes. Your clinical notes, medication protocol, and treatment goals are integrated into the patient’s Upchar Setu health record and shared with every assigned care professional. Our team works in alignment with your clinical direction — we do not override physician-directed care plans.
Your patient’s Care Coordinator flags concerns to the relevant professional and — under your agreed referral arrangement — back to your practice. For medical emergencies, care professionals call 108 immediately. For non-emergency clinical concerns, the escalation pathway is established as part of the partnership setup.
Yes. Upchar Setu’s care programs are condition and specialty-aligned. Oncologists’ patients receive the Chemotherapy Support Program with nursing, dietitian, and psychological support. Neurologists’ post-stroke patients receive physiotherapy-led rehabilitation with nursing and psychosocial support. Orthopaedic patients receive physiotherapy-intensive post-operative recovery programs. Each referral is matched to the appropriate care team composition.
Your organisation sponsors Upchar Setu care packages as a benefit for employees’ elderly parents or dependants. Employees register and access care through Upchar Setu. The institution pays — in full or as a co-pay alongside the employee — under a corporate agreement. Benefit scope, eligible family members, and payment structure are agreed directly with our partnerships team.
The corporate benefit can be configured to include annual preventive packages (Watch, Guard, Shield, Comprehensive), post-hospitalisation recovery programs, or monthly condition-specific support — depending on the benefit design your organisation chooses. All packages include MBBS doctor home visits, nursing, physiotherapy, and care coordination as relevant.
Caregiver stress is one of the leading but least visible causes of employee absenteeism and reduced productivity. When an employee is managing an elderly parent’s care without professional support — coordinating appointments, worrying between calls, taking time off for emergencies — their work suffers. Upchar Setu removes that coordination burden, provides professional monitoring that reduces health crises, and keeps the employee informed without requiring them to manage everything themselves.
Upchar Setu provides the verified care network, end-to-end coordination, and clinical documentation that insurers and TPAs need. Benefit design, coverage definitions, policyholder access pathways, and reporting formats are agreed directly during the partnership setup. We support insurer-pays, co-pay, and reimbursement models.
Upchar Setu maintains visit logs with timestamps, professional credentials on record, care notes per visit, and a full patient health record. This documentation is available in structured formats for insurer and TPA audit, claims processing, and quality review purposes.
Every Upchar Setu professional is credentialled, verified, and assigned through a managed process — not self-reported by the policyholder. Every visit is logged with timestamps and care notes. The centralised health record provides an auditable trail of every professional interaction. This accountability structure significantly reduces the fraud risk associated with informal, unverified home care providers.
A basic referral arrangement — where your team begins referring patients to Upchar Setu — can be set up within a few days of an initial conversation. A formal partnership with an MOU, agreed referral protocols, reporting formats, and integration into your workflows typically takes 2–4 weeks. Book a Partnership Call to begin.
Partnership arrangements are tailored to the model. Referral partnerships (hospitals, clinicians) are free to set up — families pay Upchar Setu directly. Corporate and insurer partnerships involve a commercial agreement. All partnership terms are discussed directly — contact us to start the conversation.
