About this role
We are looking for an experienced, responsible, and caring nurse to provide safe nursing care at patients' homes. You will assess patients, deliver clinical nursing care, monitor health, maintain records, and coordinate with doctors and care coordinators. A valid nursing registration certificate is required.

What you will do

Assess patients at home
Assess patients in their home environment as part of providing safe nursing care. Bring a careful, attentive approach to understanding their health and care needs.

Deliver responsible nursing care
Provide clinical nursing care at patients’ homes, keeping their safety and comfort central to how you work. Your role is to deliver nursing care, not simply complete a visit.

Monitor health attentively
Monitor patients’ health as part of their ongoing care. Keep the doctors and care coordinators involved informed through clear professional communication.

Maintain accurate care records
Keep records of the nursing care delivered and your patient assessments and monitoring. Reliable documentation supports continuity between visits and professionals.

Work with the wider care team
Coordinate with doctors and care coordinators so nursing care remains connected to the patient’s wider care arrangements. Approach patients and families with responsibility and compassion.

What we are looking for

Nursing experience
3–4 years of nursing experience, with a responsible approach to providing safe care in patients’ homes.

Valid nursing registration
A valid nursing registration certificate is required. Professional credentials and registration are reviewed during the network verification process.

A caring, attentive approach
You are experienced, responsible and caring, and keep the patient’s well-being at the centre of assessment, monitoring and nursing care.

Clear records and teamwork
You can maintain care records and coordinate with doctors and care coordinators so the professionals supporting a patient stay connected.
Joining the care network
Care professional roles follow a separate process from team employment. We verify, credential and onboard professionals into our home-care network.
