The role, in a few words

About this role

We are looking for an experienced physiotherapist to provide preventive physiotherapy and rehabilitation at patients' homes. You will assess patients, prepare treatment plans, improve mobility and independence, and work collaboratively with the rest of the care team. A BPT or MPT qualification is required.

Your work. Real impact.

What you will do

  1. Assess each patient’s needs

Assess patients at home to understand their needs for preventive physiotherapy and rehabilitation. Keep the focus on the person’s mobility and independence.

  2. Prepare treatment plans

Develop treatment plans based on your patient assessments. Connect the plan to the preventive or rehabilitation support the patient needs.

  3. Provide home-based physiotherapy

Deliver preventive physiotherapy and rehabilitation at patients’ homes, helping them work towards improved mobility and greater independence.

  4. Support everyday independence

Keep patients’ ability to move and function independently at the centre of your work. Approach rehabilitation as care for a person, not just a series of exercises.

  5. Collaborate across the care team

Work with the other professionals involved in a patient’s care so physiotherapy is connected to the wider care being provided.

Could this be you?

What we are looking for

  • Physiotherapy experience

2–5 years of experience in physiotherapy, with an interest in providing preventive care and rehabilitation in patients’ homes.

  • Required qualification

A BPT or MPT qualification is required. Qualifications, professional credentials and relevant registration are verified before network onboarding.

  • Patient-focused rehabilitation

You can assess patients, prepare treatment plans and focus your work on improving mobility and independence.

  • A collaborative approach

You are comfortable working with the rest of the care team and keeping your physiotherapy support connected to each patient’s broader care needs.
